- Complete TU-STEM-UP Permanent Information Form.
- Complete OK-LSAMP online Application.
- Research Ethics Certification at no cost to you. Go to the Collaborative Institutional Training Initiative (CITI Program: citiprogram.org – Learner Registration). Select the University of Tulsa as your Organization Affiliation. Complete the “Responsible Conduct of Research” training suitable for your discipline. Get a copy of the completion certificate and provided it with your TU application form.

- Identification: Name, Current Mailing Address, Tel Num, current E-mail, Citizenship and TU-ID Number.
- Academic Plans. Identify current SMET major/minor and any post bachelor work you are considering: SMET Graduate School, Business School, Medicine, Law, etc.
- Minority Group: Please identify NSF under-represented group: African American, Hispanic American, Native American (Include Tribe Roll), Native Pacific Islander (Micronesian or Polynesian) or First Generation White Female
- Essay. You must have a 200-300 words essay on your TU-STEM-UP file that is no more than two years old which discusses why you should be considered a strong candidate for admission to the program.
- Freshmen, Sophomores: Describe how you feel participation in the program will affect your long term career goals.
- Juniors, Seniors: Describe your plans after graduation and how you feel participation in the program will affect them.
- Current Academic Schedule/Performance. The sponsors of the program expect to support students majoring in a STEM field and making progress towards graduation. Scholars must consent to monitoring of academic performance starting with time receiving support through graduation in a STEM field.
- Required Meetings. Scholars are expected to attend all scheduled TU-STEM-UP bi-weekly meetings. They are also required to attend the annual OK-LSAMP-STEM conference.
- Academic Records. Please submit a current copy of your grades. Also include your latest GRE test scores. The NSF grant supporting the program requires that participants enrollments and resulting GPA be furnished to them starting when first enter into the program to graduation.
- TU Financial Aid Form. In order to ensure accurate posting of awards and to maximize benefit to the students, a copy of the current TU Financial Aid Award is requested.
- Authorizations. Scholars need to authorize the TU-STEM-UP Program Staff to review their transcripts and test scores in their TU student file. They further need to authorize the TU-STEM-UP Program Staff to release to the sponsor current enrollments and resulting grades until graduation in a STEM field.
- Deadline. The number of TU-STEM-UP Scholars awards is limited. Applications received by the first week of each semester will be considered fully. You will receive notification as soon as a decision is made on your application.
